What services does Pata Support offer?
Pata Support offers a range of personalised disability support services tailored to individual needs and goals. These include daily living assistance, personal care, community access, transport, and household tasks. We also provide support with life skill development, helping participants build confidence and independence in everyday routines.
Our team works closely with individuals, families, and support coordinators to deliver flexible and culturally respectful services that align with each person’s NDIS plan. Whether you need support at home or out in the community, we aim to make things easier and more empowering. Services can be short-term, ongoing, or adapted over time as needs change.

Am I eligible for NDIS funding?
Eligibility for NDIS funding depends on several key factors. You must be aged between 9 and 65 years, live in Australia, and hold Australian citizenship or permanent residency (or hold a Protected Special Category Visa). You must also have a permanent and significant disability that affects your ability to take part in everyday activities.
The NDIS looks at how your disability impacts your life and whether early intervention would help reduce the need for future supports. If you’re unsure, we can help you understand the criteria and guide you through the process of applying. Many people find the system overwhelming at first, so having support to clarify your eligibility can make a big difference.
What is the referral process?
The referral process with Pata Support is simple and flexible. You, a family member, support coordinator, or health professional can make a referral. Start by filling out our online referral form or contacting us directly. Once we receive the referral, our team will review the details and be in touch to arrange a meeting.
During this initial conversation, we’ll get to know your needs, answer questions, and explain what supports may be available. From there, we create a tailored support plan and begin services once you’re ready. We aim to make the process as smooth and respectful as possible with clear steps and communication from the start.
What documents do I need to prepare?
To get started with Pata Support, it helps to have a few key documents ready. These may include your current NDIS plan, any recent assessments or reports relevant to your disability, and your NDIS participant number. If you’re working with a Support Coordinator or Plan Manager, sharing their contact details is also useful.
Some people may also need to provide guardianship orders or relevant medical records, depending on the type of support needed. Don’t worry if you’re unsure, our team can guide you through what’s required and help you gather the right information. We want to make sure your supports are set up correctly and reflect your actual needs.
How do I update my support plan?
Updating your NDIS support plan usually happens during a plan review, but changes can be made earlier if your needs have shifted. If you require more support, different services, or have experienced a change in circumstances, you can request an early plan review through the NDIS.
It’s important to keep records of how your needs have changed, for example, doctor’s letters or support worker notes. If you’re with Pata Support, we can help you prepare for this by gathering evidence, updating goals, and providing supporting documentation. We’re happy to collaborate with your Support Coordinator or Plan Manager to make sure the updated plan reflects your needs and priorities accurately.
Can Pata Support work with my current Support Coordinator?
Yes, absolutely. Pata Support is happy to work alongside your existing Support Coordinator to make sure your supports are well aligned and managed effectively. Collaboration is important, and we respect the role your Support Coordinator plays in helping you navigate the NDIS. We communicate openly and professionally to ensure that everyone involved understands your goals, needs, and preferences.
Whether you’ve been with your Support Coordinator for years or have recently started working together, we’ll fit in seamlessly. We believe that strong teamwork between providers creates better outcomes and a more consistent experience for you. If needed, we’re also happy to join case meetings or planning sessions to keep everything on track.
